Creating Patient Prescription

Overview

A prescription in NICE HMS is a FHIR Composition bundle that packages ICD diagnosis codes, clinical notes, medication orders, and a follow-up appointment into a single printable document. It is created from the doctor-patient management page, not directly from the patient dashboard.

The prescription uses the doctor's personal medicine list — a library of commonly prescribed drugs that each doctor maintains. When you select a medicine, its saved frequency, route, and instruction auto-fill.

Access

  1. From the Patient Dashboard, open Quick Actions → Clinical Tools and assign the patient to a doctor.
  2. This opens the doctor-patient-management page (/admin/doctor/doctor-patient-management).
  3. Under the Prescription heading, click Create Prescription Record.

The link passes the patient ID, encounter ID, and practitioner (doctor) ID as query parameters.

The page also links to Search & Edit Prescription Record for accessing previously saved prescriptions.

Prescription Form

The form has four sections:

ICD Coding

The ECTReactComponent at the top lets you add ICD-10 diagnosis codes to the encounter. These codes appear in the printed prescription.

Notes

A rich text editor for clinical notes, chief complaints, diagnosis, and any free-text observations. Supports:

  • Editor shortcuts — pre-configured text expansions (e.g., type a short code and it expands to a full phrase)
  • Abbreviation expansion — auto-expands medical abbreviations

Follow-up Appointment

A date picker to schedule the next visit. This creates an appointment record linked to the prescription.

Medication Request

Add one or more medicines to the prescription. Each medicine line has:

Click Add to add the medicine to the prescription. Added medicines appear as rows below — each can be edited or deleted.

Doctor's Medicine List

The medicine select dropdown pulls from your personal medicine library, managed through the DoctorMed component. When you select a medicine that's already in your library, its stored frequency, route, and instruction auto-fill in the form fields.

If you type a medicine name not in your list, it's added as a new entry with whatever values you enter for that prescription. To manage your full medicine list, use the DoctorMed panel above the medicine entry row.

CDSS AI Assistance

The doctor-patient-management page includes a CdssMaster component that provides Clinical Decision Support:

  • Drug interaction checks
  • Dosage recommendations
  • Diagnosis-based medication suggestions (using modules for diabetes, hypertension, lipids, and lifestyle)

This appears alongside the prescription links, not inside the prescription form itself.

Delivery

After submitting the prescription, the DocumentHOC wrapper provides delivery options:

Editing and Reusing Prescriptions

To modify an existing prescription or reuse it for a follow-up visit:

  1. Go to Search & Edit Prescription Record from the doctor-patient-management page.
  2. Find the prescription by patient ID.
  3. Open it — the form loads with the saved data.
  4. Modify as needed (change medicines, adjust notes, update follow-up date).
  5. Submit — a new prescription is saved; the original remains unchanged.

This serves the same purpose as a "Clone" feature — reuse a previous prescription as a starting point for the current visit.

FAQ

Q: How do I add a new medicine to my personal list? A: Type the medicine name in the Medicine select and fill in the details. It's saved to your list when you add it to a prescription. Alternatively, use the DoctorMed panel to manage your list directly.

Q: Can I save prescription templates? A: There is no separate template feature. Use Search & Edit Prescription Record to open a previous prescription, modify it, and submit as new.

Q: How do patients receive prescriptions on WhatsApp? A: After submitting, select the WhatsApp delivery option in the DocumentHOC toolbar. The prescription PDF is sent to the patient's registered mobile number. Ensure the number is correct in the patient's record.

Q: Does the prescription include ICD codes? A: Yes. The ICD codes added via the ECTReactComponent appear on the printed prescription.
